If you require any vaccinations relating to foreign travel you need to complete the travel form below at least 6 weeks before you are due to travel. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to complete the form as early as possible - at least 6 weeks before you travel - as an appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
PLEASE BE AWARE THERE MAY BE A SHORTAGE OF CERTAIN TRAVEL VACCINES AND WE MAY BE UNABLE TO OFFER THEM. COMPLETE THE TRAVEL VACCINE FORM AND CALL THE PRACTICE IN ONE WEEK TO CLARIFY IF ANY VACCINES ARE REQUIRED AND IF THEY ARE AVAILABLE.
Anti-malarial drugs can be purchased from any of the local pharmacies who can advise on requirements for area of travel.
If travelling at short notice you should attend a Specialist Travel Clinic who will be able to provide a more timely service.
